'At the grave of my breast with your ancient grief / Sleep peacefully — O mad, mad heart' — the breast as a grave is a sustained metaphor: the heart has lived inside the speaker's chest like a tenant in a tomb, carrying ancient (kohan, old, worn) grief. To tell it to sleep peacefully in this grave is to grant it rest after a life of agitation. The 'mad, mad heart' (dile divane divane) is addressed with almost affectionate repetition — the madness is acknowledged without being condemned.
